The SVCHC professional team works closely with community health and social service partners in order to be linked with those non-medical services needed by clients. The Team consists of Physicians, Nurse-Practitioners, Registered Nurses, Social Worker, Health Educator/Promoter, Community Health Worker, and others, and it targets issues impacting the local community’s health through health promotion and community development programs.
