Ages: 16 year(s) and up Youth; Young Adults; Adults; 16yrs and up; with a serious mental illness; the individual must be able to handle his/her own medication, not abuse alcohol or drugs while in the program, and be able to function within a group house setting

Provides intensive case management support geared to meet the unique needs of all participants/clients. Individuals work with a case manager to discover their strengths, determine areas of need and develop support plans which may include skill development, planning daily/weekly activities, accessing medical services, grocery shopping, grooming and hygiene and dealing with income and housing issues. Services are located in the Barrie area.
